1. Applicant provides name and checks yes to be a guest. (Being considered as a guest on the podcast means you are open to physically talking on the podcast for an episode.)

2. Applicant provides name and checks no to be a guest BUT is open to having the host read their testimony via paper.

3. Applicant does NOT provide name and checks NO to be a guest BUT is open to having the host read their testimony via paper.
